All documents listed below must be on file at Holy Family Academy before an application is considered complete.

• Official transcripts from all colleges and universities attended. Copies of transcripts may be used for application purposes. If hired, you will be required to provide official original transcripts sent directly from the colleges/universities to the school. Transcripts are NOT required for Substitute and Support Staff applicants.

• PDF of teaching certificate(s) (if applicable) upload when submitting application.

• Copy of teacher service record (if applicable) upload when submitting application.

• Pre-Employment Affidavit DOWNLOAD, complete and upload when submitting application.

• 2 Letters of Recommendation

Applications and the documents submitted shall become the property of Holy Family Academy of Van Zandt County. Any information submitted with or in support of an application may be revealed to any persons who may participate in the selection of employees.

Applications will be placed on file and considered if and when vacancies occur in an appropriate field. Your application will remain active for two years.
